Family medicine is the center of primary care. Unlike other specialties that are limited to a particular organ or disease, family practitioners are the only specialists qualified to treat most ailments and provide comprehensive health care for people of all ages.

We also accept SELF-PAY as an option to obtain care from Amend. We are awaiting approval for BCBS ND Medicaid Expansion Plan. We will add this plan to the list once approved.

The choice to go to the emergency room can depend upon how severe the symptoms are. Emergency departments are set up to treat the most severe symptoms like choking, difficulty breathing, neck/spine injuries, severe burns, severe chest pain or pressure, seizures and more.

Items you should bring to your appointment include:

  • your insurance card
  • your state-issued ID (driver’s license)
  • a list of your medications

We appreciate when patients bring their medications into the visit with them, if able. This allows us to get a full understanding of what doses and types of medications you are taking. Any past medical, surgical, or family history you wish to bring with you is appreciated as well. 

Absolutely! We are able to provide copies of your immunization records at your request. You can request these by calling our office or asking for them at your next visit. 

You can access your medical records through the Practice Fusion patient portal. Once you are established with us at the clinic, you will receive an email invitation to sign up for the portal. This can be accessed from your phone or computer. You can talk to the clinic staff if you need to access hard copies of your records or need them sent to another location for any reason. You are able to sign a Release of Information (ROI) through the clinic if needed. 

No, there is no cost to access your records.
